Responsibilities:
- Provide case management services to particularly vulnerable individuals and families - Conduct assessments to determine client need for enrollment in services and develop appropriate service plans - Collaborate with clients to set goals and develop strategies for achieving them - Connect clients with community resources and support services - Monitor client progress and make necessary adjustments to service plans - Advocate for clients and ensure their rights are protected - Maintain accurate and up-to-date case records Skills:
- Proficiency in motivational interviewing techniques - Understanding of behavioral health principles and practices - Ability to effectively manage crisis situations - Strong background in social work or related field As a Case Manager, you will play a crucial role in supporting vulnerable individuals and families.
